Things to Know Before Your First Visit

At the time you schedule your appointment, a member of our staff will be sending you an email with forms you can fill out online that will upload directly to your chart. These forms need to be filled out online prior to your appointment. Otherwise, you risk losing time with your Physician and/or your appointment all together. Filling these forms out prior to your appointment will make sure your appointment goes as smoothly as possible and will ensure you have adequate time to spend with the physician. You will also be able to sign up for the secure patient portal through the email or with the button below. It’s easy to use and will allow you to have access to your entire chart, future appointments, as well as superbills if you wish to submit anything to insurance.
